
To direct, develop, implement and coordinate medically necessary treatment services withinPartnership’s Health Services for adults and children with behavioral health and/or substance usedisorder needs.
▪ Performs risk assessments for members seeking access to behavioral health or SUDservices.▪ Collaborates and coordinates care as part of the multidisciplinary team to evaluate andadvocate for the medical, behavioral and psychosocial needs of the member whilepromoting quality and cost-effective outcomes. Acts as a support/resource to theBehavioral Health Access team and others within the organization for cases that arewithin appropriate scope of practice.▪ Maintains a high level of clinical expertise. Is knowledgeable of current clinicalconditions, methods, medications, theoretical frameworks and interventions.▪ Utilizes best practices in delivery of behavioral health services in accordance withprofessional and clinical guidelines.▪ Coordinates with behavioral health providers, psychiatrists, drug and alcohol counselors,case managers, and other support staff to ensure continuum of care.▪ Reviews and processes Single Case Agreement requests in accordance with establishedguidelines and workflows.▪ Monitors and responds to correspondence in behavioral health inbox in accordance withestablished guidelines and workflows.▪ Maintains accurate and timely documentation, records, and case files in the PartnershipCase Management System for members in case management.▪ Attends and participates in multidisciplinary rounds, case conferences, and inter-agencymeetings and may lead these meetings when appropriate.▪ Participates in developing procedure and informational materials that support Partnershipservices and benefits.▪ Conducts and contributes to special training for staff, professionals, public and privateagencies and others as identified.▪ Develops and maintains knowledge of community-based resources and organizations.▪ Aids members to connect with community-based organizations to support and enhancewellness.▪ Communicates clearly and effectively through all mediums of communication withmembers, providers, vendors, community partners, and Partnership employees.▪ Assists call center staff with enhancing the member experience, which may includesupporting access to care and ensuring closed loop referrals.▪ Performs other duties as assigned.
Education and Experience
A Master’s Degree as a Clinical Social Worker or Master’s Degree thatqualifies for licensure as Marriage and Family Therapist. Two (2) yearsof experience working as a Licensed Clinical Social Worker, LicensedMarriage and Family Therapist, or Licensed Professional ClinicalCounselor. Experience in the delivery of care and services that addresssocial, psychological and physiological needs of individuals fromdiverse populations. Demonstrated skill in using evidence-basedprinciples, methods and techniques to address mental health andsubstance use disorders across multiple care settings.
Special Skills, Licenses and Certifications
Possession of a current Licensed Clinical Social Worker (LCSW)License, Marriage and Family Therapist (LMFT) License, or LicensedProfessional Clinical Counselor (LPCC) issued by the California StateBoard of Behavioral Science Examiners OR possession of successorlicensure recognized as equivalent by the State of California.Considerable knowledge of prevention, treatment and rehabilitationtechniques used in the identification and treatment of mental illness andemotional disorders. Federal, state and local laws, rules and regulationsassociated with mental health treatment and education programs. Indepth understanding of the treatment of mental illness, includingwellness and recovery as well as strength - based approaches forindividuals and their families. Valid California driver’s license andproof of current automobile insurance compliant with Partnershippolicy are required to operate a vehicle and travel for companybusiness.

Ability to work within an interdisciplinary structure andfunction independently in a fast-paced environment while managingmultiple priorities and meeting deadlines. Ability to apply clinicaljudgment to complex medical situations and make quick decisions.Effective telephone and computer data entry skills required. Experiencein managed care business practices and ability to access datainformation using computer systems. Excellent English written andverbal communication skills.
Work Environment And Physical Demands
Daily use of the telephone and computer for most of the day. Standardcubicle workstation. Must be able to lift, move, or carry objects ofvarying size, weighing up to 10lbs.
