Duties and Responsibilities:/
1. Provides individual, group, conjoint or family psychotherapy and supportservices to children with severe emotional disturbance and their family.
2. Conducts clinical assessments, develops diagnoses, and participates indeveloping person/family centered treatment plans with client families. Monitorsclient progress and coordinates appropriate behavioral health and other neededservices including, but not limited to, community coordination, outreach andreferrals.
3. Responsible for meeting all requirements for Home-based hours (minimum 6hours per family per month including at least 4 hours face-to-face).
4. Responsible for ensuring full compliance of clients on caseload have up-to-date/current CAFAS/PECFAS on ongoing basis.
5. Responsible for maintaining an electronic and paper chart which includesrequired documentation of services provided, progress notes, treatment plans,periodic reviews and client rights documents.
6. Participates in individual supervision and team meetings to review progress,identify barriers and develop strategies for better assisting families in reachingtheir goals.
7. Provides crisis intervention on an on-call rotation basis to help offset potentialrisk of self-harm or conditions which could lead to out of home treatment.
8. Ensures that paperwork is maintained in accordance with funder requirementsas well as Easterseals MORC policies and procedures.
9. Responsible for assessing, reviewing all requests for services and authorizingmedically necessary services for all consumers. Only implements therapeuticmodalities approved by Easterseals MORC Michigan.
10. Willingness to support and assist consumers with trauma related issues.
11. Ensures that all individuals served caseload have CAFAS/PECFAScompleted initially, quarterly, annually and upon discharge.
12. Ensures that all individuals served caseload have CAFAS/PECFAScompleted quarterly.
