Job Description

POSITION: Behavioral Health Clinician

DEPARTMENT: Behavioral Health

LOCATION: 1575 Blue Hill Avenue, Mattapan, MA 02126

REPORTS TO: Behavioral Health Director /BH Clinical Manager

TYPE: Full-Time

SALARY: Negotiable

This position shall have primary responsibility for completing behavioral health services under the general direction of the Behavioral Health Clinical Service Manager in collaboration with the Director of Behavioral Health Services, in accordance with policies, procedures and protocols established by Mattapan Community Health Center. In addition, the incumbent is required to follow all other appropriate standards of practice, licensing and certificate, and other regulatory agencies’ requirements.

RESPONSIBILITIES

1. 90%

  • Clinical Care
  • Perform appropriate psychosocial assessments and interventions.
  • Perform short- and long-term outpatient therapy.
  • Create treatment plans and refer clients to appropriate services based on assessment and treatment plan needs.
  • Assist in the implementation of community and social programs as a professional clinician in accordance with Mattapan Community Health Center grants/contracts with federal, state, and local agencies and mandates of service requirements with those grants/contracts.
  • Support medical providers with connecting clients to appropriate behavioral health services.
  • Perform crisis assessment and crisis intervention and review high risk situations with clinical supervisor.
  • Prepare appropriate transfer, discharge plans, and assist clients in a therapeutic transition from services.
  • Prepare psychosocial educational materials for clients, as needed, or required.
  • Engage with clinical, health education, community health and other staff members in the provision of services to patients. Provide referrals for services needed.
  • Use tests, measurements, and measurement scale (PHQ2/PHQ9, GAD7, Vanderbilt Scale, etc.) for general and specific assessments as well as objective patient criteria in the provision of services and evaluation procedures.
  • Create patient center plans, provide ongoing treatment, assess patient’s ongoing clinic needs and progress, and provide consultation and coordinate care with other designated team members.

  • Attend bi-weekly Integrated Team Meetings
  • Attend monthly behavioral peer support meetings
  • Attend weekly supervision
  • Prepare patient reports, correspondence, referrals and other materials for community agencies, schools, legal proceeding, and others as needed.
  • Maintain and complete in a timely manner all appropriate clinical paperwork and documentation in compliance with the Behavioral Health department policy and procedures, including use of the EHR and use of DSM 5 coding as needed.
  • Participate in risk management meetings, if applicable to case.
  • Perform other duties as required by the position or as assigned by the BH Director. Must cooperate fully with MCHC’s Corporate Compliance Program and performs all tasks as assigned by the Director of Behavioral Health, CMO and/or CEO.

Requirements

Master’s in Clinical Social Work (LCSW or LICSW) or Master’s in Mental Health Counseling (LMHC). Current, valid, and unrestricted licensure to practice in MA.

Experience in an ambulatory setting is preferred. Minimum of one to two years of relevant clinical experience.

Experience working with all age category (children, adolescent, and adults).

Successful candidate must be a self-starter, capable of working in a team based approach model, highly motivated, and able to respond to same day visits and/or crisis.

Organizational skills and comfort working in a team-based, collaborative setting are a requirement.

Bilingual in English and Haitian-Creole or Spanish is a plus.

Effective communication skills with staff and patients.

Microsoft outlook and EMR experience preferred

Familiarity with multicultural groups, previous work experience in a multicultural setting and willingness to remain current in field of focus is preferred.
