Job Description

ESSENTIAL JOB RESPONSIBILITIES:

  • Will provide Case Management Services to a caseload of approximately 25 Participants.
  • Will apply the Housing First approach to serving people with housing and other service needs.
  • Will submit routine reports and outcome data, per contractual requirements, and will utilize Dr. Cloud
  • Will conduct necessary assessments, complete mandated legal documentation, and complete Individualized Service Plans (ISP) according to funding source and policy timelines.
  • Will plan, schedule, and coordinate meetings with Participants, and facilitate route home visits as required by stated performance measures.
  • Will research, access, and link to appropriate medical, psychiatric, education, community, and other social services/resources, as indicated by assessments and individual participant preferences.
  • Will communicate and work collaboratively with other providers, leasing managers, and/or landlords to ensure appropriate service provision.
  • Will meet with the supervisor for regularly scheduled meetings and as assigned by the supervisor.
  • Will attend and actively participate in regularly scheduled staff meetings as assigned by the supervisor.
  • Will attend and actively participate in required training, as assigned.

  • Monitoring – Monitor participant progress and respond to unmet needs; able to monitor provider service provision and billing practices and ensure accuracy and efficiency.

  • Coordination – Adjust casework to coordinate efforts with other agencies to ensure consistency and adequate follow-up.
  • Action Orientation – Take initiative rather than passively accepting; take action and attempt to influence events to achieve goals.
  • Work Commitment – Willingly commits extra effort and time to get a job done; identifies with organization goals; exhibits excellent time management abilities.
  • Oral and Written Communication Skills – Ability to exhibit appropriate use of grammar and spelling in written communication, exhibit appropriate use of grammar in oral communication; ensure oral and written communication is professional.
  • Building Relationships – Able to establish rapport with participants, Easterseals’ staff and administration, program funders, and provider agencies.
  • Team Playing – Work cooperatively with other Easterseals Case Managers/Supervisors.
  • Mission Advocacy – Ability to identify with project and Easterseals objectives; adequately, constructively, and directly communicate these objectives.

Requirements

QUALIFICATIONS:

  • Bachelor’s Degree in Social Service or Human Services related field.
  • Must have their own reliable transportation.
  • Willing to travel during the day to outlying parishes as required.
  • Able to multitask and meet deadlines.
  • Excellent written and verbal communication skills.
  • Proficiency in the use of Microsoft Office Software (i.e. Word, Excel) and computer literate.
  • Must be a team-player.

PHYSICAL REQUIREMENTS with or without reasonable accommodations:

  • The employee is regularly required to operate a computer, file and retrieve written documents,
  • communicate with others on the phone and in-person.
  • The employee is frequently required to walk, sit, use hands, and lift and/or move lightweight items.
  • Ability to move independently within the facility and community.
  • Must be able to travel and meet with participants

WORKING CONDITIONS:

  • Work is performed in a normal working office setting that is environmentally controlled and out in the field
