As a Behavioral Health Case Manager, you'll be the advocate, connector, and champion that vulnerable children and families need. You'll work directly with youth experiencing serious emotional and behavioral challenges, helping families navigate complex systems, access services, and create pathways toward stability and success. This is not a desk job. You'll be in homes, schools, communities, and care settings—building relationships, solving problems, and helping families move from crisis to confidence.

What You'll Do

  • Conduct comprehensive assessments to identify client and family needs and develop individualized service plans that drive meaningful outcomes
  • Connect children and families to critical community resources and treatment services
  • Advocate for clients across schools, healthcare systems, behavioral health providers, and community organizations
  • Provide in-home support and guidance to families
  • Document progress and outcomes through our electronic health record system
  • Participate in family service planning, case reviews, and community outreach efforts

Job Qualifications Experiences:

  • Bachelor's Degree Required in Human Services/Social Work or related degree. Candidates with degrees outside Human Services may also qualify with additional relevant experience.
  • Minimum 1 year working with severely emotionally disturbed children (or 3 years if degree is outside of Human Services)
  • Florida Driver's License with access to a reliable, insured vehicle
  • Can work Mon-Fri, 8-5pm, with flexibility to work outside standard hours when needed
  • Completion of AHCA-approved Targeted Case Management certification training within three months of hire

***The bonus will be paid in two installments with the first being after successful completion of your 90-day orientation period and the second installment being after your 12 months of service. This sign-on bonus is taxable; all regular payroll taxes will be withheld. In the event that you leave CHS within 12 months of your date of hire, you will be responsible for reimbursing CHS for the sign-on bonus at a pro-rated amount.
