Job Description

Salary: $65,000 - $70,000

PRIMARY JOB FUNCTION(S):

  • Attend work regularly according to assigned schedule and in accordance with agency policy
  • Work cooperatively with others including all staff, supervisors, and administrators
  • Maintain ongoing communication with all involved staff
  • Conduct Functional Behavior Assessments
  • Develop Behavior Support Plans (BSP) in accordance with State regulations
  • Develop and assist in the implementation of behavior and /or reinforcement programs
  • Create and monitor behavior data collection systems and analyze results
  • Present BSP for approval through the Peer Review of Behavior Intervention Strategies (PROBIS)
  • Present BSP to the Human Rights Committee (HRC) as needed
  • Submit documentation in a timely manner
  • Effectively manage a caseload and meet billing requirements
  • Monitor required documentation regarding behavior incidents and interventions
  • Conduct BSP training for Direct Support Professionals (DSPs) and related support personnel in the day program and/or residential settings
  • Participate in a variety of scheduled activities and required meetings
  • Serve as a liaison between Chimes and community agencies
  • Attend psychology, psychiatry, and counseling appointments for service recipients
  • Serve as a liaison between the psychologist and/or counseling providers, service recipients, and related staff
  • Maintain documentation in accordance with State law and HIPAA regulations
  • Evaluate and monitor individual and/or group programs focusing on independent living skills, personal career, socialization, and areas deemed relevant for the development of each service recipient
  • Initiate, secure, and monitor support services in relation to clinical and/or related services
  • Chairs and participates in Interdisciplinary Team Meetings to ensure that the goals and recommendations of these meetings are properly developed and accurately documented in the consumers’ record
  • Review monthly and quarterly progress and services plans for consumers and documents on appropriate forms
  • Serve as the primary contact person for parents/guardians regarding program related issues
  • Assist consumers and families in social and behavior adjustment
  • Other duties as assigned

REQUIREMENTS:

EDUCATION:

  • Master’s degree from an accredited university in behavioral or social science or related field, or Board Certified Assistant Behavior Analyst (BCaBA), or Master of Social Work (MSW), or Licensed Clinical Social Worker (LCSW)

EXPERIENCE:

  • Demonstrated competency in developing, implementing and monitoring behavioral programs.
  • Experience in crisis management in an environment similar to that to which they are assigned
  • Must have an acceptable driving record as determined by criteria established by the Agency’s insurance carrier and policy
