Job Description

The Assistant Wellness Director plays an integral role in upholding American House’s mission to enrichthe lives of those we serve. The Assistant Wellness Directors aid in the supervision, education, directionand support all Wellness team members. This is a leadership position within the community and servesas manager on duty in the absence of the Wellness Director.

This role is open due to an internal promotion. On Call required every other weekend.

Experiece with scheduling and Memory Care strongly preferred.

Duties/Responsibilities:

• Coordinate staffing schedule of Resident Assistants and Med Techs• Participate in continuous quality improvement efforts by ensuring proper documentation andrecord keeping is maintained• Participate in care plan development and implementation through collaborative efforts withresident responsible parties, physician, third-party care team, and Wellness Director• Prepare and administer medications as allowed by state regulation to residents followingestablished policies and procedures as needed• Observe residents and report changes in resident’s psychosocial and physical condition as wellas any concerns, e.g., skin breakdown, rash, behavioral changes, speech change, change inappearance, etc., to Wellness Director.• Ensure a clean, safe, and orderly environment for residents. Ensure resident rooms are cleanand tidy throughout each day scheduled housekeeping tasks such as laundry are completed• Assist residents with activities of daily living, to include ambulation, transferring, showering,personal hygiene, incontinence care, feeding, reminders, and supervision according to theindividual plan of care.• Gathering, engaging, and assisting residents in activities, conducting activities in groups or oneon-one with residents as requested. Encouraging residents to socialize and participate inplanned activities and develop friendships with other residents.• Communicate professionally with physicians and third-party specialists, such as homecareproviders, lab providers and hospice professionals• Follow established safety protocols, including fire protection, infection control, blood bornepathogens, hazardous waste, and quality assurance procedures. Respond to all requests forassistance promptly and with a positive demeanor including emergency pull cords, telephonecalls, and requests from family members or friends.• Report all complaints, grievances, suspicion of abuse and instances of abuse to the WellnessDirector and/or the Executive Director.• Participate and attend Wellness meetings and required trainings to ensure compliance with allregulations and policies• Supporting Wellness Director with supervisory responsibilities of all wellness team membersincluding interviewing, hiring, and training employees; planning, assigning, and directing work;appraising performance; rewarding and disciplining employees; addressing complaints andresolving problems.• Assess/evaluate potential residents for Assisted Living/Memory Care level of care utilizingassessment tool. This may involve assessing/evaluating potential residents in their home oranother healthcare setting.• In the absence of the Wellness Director, ensure compliance with all state regulatoryrequirements including mandatory reporting to State Regulatory Body.• As delegated by the Wellness Director, ensure that all incident reports are completed, filed, andthat follow-up is conducted appropriately.• As delegated by the Wellness Director, ensure that all staff follows established safetyregulations, including fire protection, infection control, blood borne pathogens, hazardouswaste, and quality assurance programs.• As delegated by the Wellness Director, maintain compliance with state regulations and policiesfor medication administration including oversite of medication management, validation oftimeliness and completeness of medication records, completing required responsibilities forMedication Aide programs via supervision, delegation, training, and ongoing competencychecks, where applicable.

Preferred Skill/Abilities:

• 3 years of management experience in healthcare or service-related setting• Experience working with Seniors and individuals with memory loss• Licensed Practical Nurse or Registered NurseEducation and Requirements:• Certified Nursing Aide and Certified Med-Tech in the appropriate state• Minimum 18 years of age

• Must pass a pre-employment background check, drug screen, TB test and physical

Physical and Mental Demands:

• Ability to regularly stand, talk, bend, stretch, pull or push and lift items• Ability to help others with daily activities including but not limited to; bathing,dressing/undressing, dining, exercising, etc.• Ability to frequently communicate with residents, families, co-workers, etc. Must be able toexchange accurate information in given situations.• Specific vision abilities required by this job include close vision, depth perception, and the abilityto adjust focus.
