Job Description

$57,960 - 66,654 /Yr.
Schedule: Monday - Friday; 8:00 am - 4:30 pm

Location: Rogers Park

The Assistant Team Leader is responsible for assisting the Team Leader in managing the day to day functions of the team.  These duties will vary depending on the needs of the team, but may consist of reviewing paperwork and documentation, team scheduling, providing consultation, training, crisis management, etc.  The Assistant Team Leader will also maintain a reduced caseload and perform daily duties of a Recovery Counselor. They will provide and ensure the team provides client-centered, strengths-based, and trauma-informed services to clients with severe mental illness and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach. 

Responsibilities:

Clinical Caseload:  

  • Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.   
  • Coordinate the establishment of an individual recovery plan with the client, client’s support system, and other care providers. 
  • Recognize and act on opportunities to move clients to appropriate levels of care; provide referrals to additional services as indicated. 
  • Assist clients in identifying signs and symptoms of de-compensation.  Work collaboratively with clients to identify coping skills to assist in managing symptoms and stressors experienced. 
  • Assist clients in learning and improving independent living skills; i.e. personal hygiene, housekeeping skills, nutrition, and shopping for food and personal items 
  • Educate and assist clients with applying for entitlements; i.e. Supplemental Security Income, Social Security Disability Insurance, Medicare, Medicaid, and LINK 
  • Accompany clients to important appointments in the community and provide support with issues related to housing, substance use, budgeting, social support, and medication. 
  • Advocate on client’s behalf and empower clients to advocate on behalf of themselves when appropriate 
  • Communicate effectively within the team model for a multi-disciplinary approach to client care. 

Administrative: 

  • Serve as the team Supervisor in the absence of the Team Leader 
  • Support team leader in running team meeting and run meetings in absence of TL
  • Assist team with scheduling appointments as needed and in prioritizing client needs 
  • Assist with monitoring for the completion of required documentation (e.g. IM+CANS)
  • Assist team leader in communicating with the financial services department to finalize clients’ rental payments, BMFs, and make adjustments as needed 
  • Assist team leader with onboarding, training, and educating new employees on the team to provide quality services to clients  
  • Assist team with client crisis management (I.e. conduct 2-person home visits, complete petitions, etc.) as directed 
  • Perform other related duties and/or projects as assigned by TL and/or PM

Qualifications:

  • Must have one of the following requirements: 
    • MHP designation, either through a Bachelors in a related field, any Bachelors degree with two years of experience or a High school diploma with 5 years of supervision in field such as counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field 
    • Masters’ Degree in Social Work, Psychology, or Counseling other closely related field preferred 
  • Valid Illinois driver’s license with daily access to a well-maintained vehicle with $100,000/$300,000 liability insurance   
  • Experience working with people with mental health and/or substance abuse diagnoses preferred 
  • Knowledge of ACT team model preferred
