Key/Essential Functions & Responsibilities

  • Assists Program Supervisor in planning of all logistics for staffing and maintaining coverage, manages staff schedules, assists in managing the food, household, and recreation lines in the BHS budget, among other leadership responsibilities.
  • Supports BHS home staff, providing leadership to the program, in the absence of the Program Supervisor.
  • Ensures the appropriate, therapeutically documented level of supervision in the assigned program at all times to protect the health, safety and habilitation of the people we support, consistent with agency philosophy and department practices and in line with Federal, State, Local and regulatory requirements.
  • Assists with the coordination of the interdisciplinary team, promoting and maintaining the excellence of the program and ensuring implementation of person-centered treatment and behavior plans and coordination of medical care, medication administration and other necessary services
  • Directly supervises DSP/RBT staff, serving as a role model for the DSP/RBT staff, providing guidance and support
  • Participates in the hiring process, interviewing and selecting qualified staff based on program needs.
  • Assesses work performance and ensures quality service provision through ongoing staff supervision and observation
  • Serves as Administrator-on-Call (Available/On Call Supervisor )with other program leadership on an as needed basis and provides program/floor coverage, when necessary.
  • Serves as Transition Navigator for the BHS Home, ensuring a comprehensive and person-centered approach to treatment and planning for continued success following discharge for each resident through:
  • Planning each person’s admission to and discharge from the BHS home and establishing and monitoring necessary services and providers.
  • Facilitating scheduling of general and psychiatric assessments and accessing medical and clinical providers.
  • Performing check-ins with staff on at least a daily basis to discuss the progress of people being supported and acquire updates for transition planning.
  • Facilitating weekly treatment team meetings to discuss each person’s stay at the home, planning for successful transition on an ongoing basis
  • Maintaining documentation of meetings and progress, including attainment of performance deliverables during the person’s time at the and discharge/transition plans.
  • Establishing post-transition supports and coordination including regular check-ins at predetermined intervals following discharge

Requirements

  • Experience working with individuals with developmental disabilities is preferred.
  • Strong management skills with the ability to lead and motivate a team.
  • Excellent relationship management skills to foster positive interactions with clients and families.
  • Background in behavioral health or social work is a plus.
