Job Description

Proposed Schedule: 40 hours per week. Shifts may vary based on the operating hours of the Health Center.

The Assistant Nurse Leader, Registered Nurse (RN), provides daily leadership and coordination of Health Center nursing operations while embracing the Core Values of Quality, Appreciation, Creativity, and Sustainability in pursuit of the HHHN Mission and Vision. This leader partners closely with the Nurse Leader (NL) and Practice Leader to ensure reliable patient flow, optimal use of nursing resources, and delivery of exceptional patient care as measured by patient satisfaction and feedback. The Assistant Nurse Leader supports team-based care, helps achieve annual health center and Network performance goals related to quality, compliance, and financial outcomes, and assists with recruiting, training, and retaining qualified nursing staff.

This position plays a key role in fostering staff engagement, patient satisfaction, problem-solving, and implementing sustainable care models while collaborating across the Network toward common goals and benchmarks for success.

Essential Duties and Responsibilities:

  • Promote and continuously monitor patient satisfaction by providing coaching and positive reinforcement and leading by example in creating a patient-centric culture.
  • Respond promptly to patient feedback and ensure resolution striving for 100% satisfaction when feasible and hold team members accountable for follow-through.
  • Function as a liaison between patients, staff, and providers to facilitate communication regarding patient needs and perform frequent rounds across locations as directed.
  • Provide a consistent level of high-quality care and exceptional customer service while achieving productivity and efficiency targets.
  • Assist the NL in day-to-day nursing team oversight, ensuring patient flow is maximized, and staffing is aligned with volume and acuity.
  • Remain accessible in the center during operational hours to provide real-time coaching, oversight, and support for nursing staff and patient flow.
  • Prepare and/or support staffing schedules (including time-off requests) to ensure adequate coverage and review schedules to confirm staffing is in place.
  • Participate in candidate interviews, nursing team onboarding, and offboarding in conjunction with the NL and Practice Leader.
  • Provide regular feedback to the NL on staff performance, redirect and coach staff as indicated, and contribute to performance reviews and improvement plans.
  • Mentor and guide new and incumbent staff based on skill set by providing ongoing support and development utilizing available resources and collaborating with key stakeholders such as Nursing Administration and Human Resources team.
  • Ensure timely and accurate communication of patient care updates, workflow changes, and operational issues to the NL, supporting effective decision-making and continuity of care.
  • Review incoming reports promptly and accurately, complete all required sections, and ensuring they are returned promptly to maintain compliance and support operational efficiency.
  • Maintain a hands-on approach by working in the capacity of a clinical RN performing nursing duties as needed.
  • Monitor clinical inboxes, encounter flow, nurse visit schedule, and all nursing team tasks to ensure staff are working to the highest level for their license and/or scope for duties.
  • Ensure proper operation of medical equipment; perform monthly and quarterly environment of care (EOC) audits accurately.
  • Maintain nursing supply inventory by checking stock, determining appropriate inventory levels, anticipating needed supplies, placing orders, and verifying receipt of supplies.
  • Assist with the support and oversight of standards, quality measures, clinical competencies, policies and procedures, point of care testing (CLIA), specimen collection, and proper documentation in the electronic record.
  • Assist the NL with budget preparation and monitoring, including reviewing monthly reports and explaining variances.
  • Help manage staff time approvals in ADP, mileage verification, and compliance with organizational policies.
  • Support data collection and reporting for quality metrics, patient satisfaction, and operational performance indicators.
  • Prepare agendas, assist in facilitating nursing staff meetings, and document and follow up on action items.
  • Coordinate with HR and Nursing Education for competency tracking, annual evaluations, and training compliance.
  • Maintain accurate records for EOC audits, supply inventory, and equipment maintenance logs.
  • Assist with policy updates and communication to ensure staff awareness and adherence.
  • Participate in regional and network-wide nursing leadership meetings and contribute to strategic initiatives.
  • Work in other Health Centers when there is a staffing need.

Qualifications

  • Registered Nurse with an A.A.S. degree or higher from an accredited school of nursing
  • Current and unrestricted NYS RN license
  • Minimum of 2-3 years of clinical RN experience preferred, at least 1–2 years of supervisory/lead experience preferred, ambulatory care and emergency/urgent care experience is a plus
  • Ability to travel throughout the Network as needed
  • Current BLS certification
  • Excellent verbal and written communication skills; ability to communicate effectively with care team members, patients, and external contacts
  • Demonstrated ability to mentor, coach, and build an engaged, productive team
  • Strong organizational skills, attention to detail, and decisiveness with a willingness to take initiative and implement innovative solutions
  • EMR proficiency and computer competencies; proficiency with Microsoft Office Suite
  • Knowledge of current nursing practices, standards of care, and team-based primary care models
  • Self-directed, motivated, and able to work independently and collaboratively; ability to connect nursing operations to broader Network goals (big picture orientation)

The pay rate for this position is $84,000/annually.
