SCOPE AND PURPOSE OF POSITION:

The Assistant Director of Care Management will provide direct supervision to Health Home Care Management Supervisors and will assist the Director of Care Management in developing and implementing departmental plans and goals, ensuring compliance with both Lead Health Home regulations and NYS Department of Health regulations for Health Home Care Management.

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Conduct weekly/bi-weekly supervision with care management supervisors/senior care managers to provide support, direction, and leadership development.
  • Facilitate monthly team meetings with subordinates.
  • Assist in training new employees.
  • Ensure that supervisors and care management staff are attending all trainings as required and remain updated with any needed certifications (i.e., CANS)
  • Complete comprehensive auditing of care management charts to ensure compliance with Health Home policies and ensure timely follow-up by care managers and supervisors.
  • Responsible for distribution of compliance data from tableau reporting system to care management staff.
  • Attend all monthly training/update webinars and office hours for Lead Health Homes.
  • Perform supervisor duties in the event of their absence.
  • Oversee billing to ensure that care managers are submitting all billing questionnaires by the last day of each month and are billing at capacity.
  • Work with Corp Officer leadership and Supervisors to strengthen the integration of health home care management into local Salvation Army activities.

Qualifications

SPECIAL SKILLS, CERTIFICATES, LICENSES, REGISTRATIONS

  • Bachelor’s Degree (BA) and minimum of six years of experience minimum, master’s degree preferred. Relevant experience includes serving children and families in child welfare, developmental disabilities, behavioral health, primary care or social services. Experience in health home care management preferred. Must have 2-4 years of supervisory experience.
  • Work competently and with professionalism across a broad spectrum of individuals/families with diverse cultural, ethnic, and racial backgrounds and experiences; Ability to deliver family-driven, client-guided services.
  • Ability to work in an effective and focused manner when self-directed and beyond immediate oversight, be highly organized and self-motivated. Must be able to work independently and deliver upon goals and outcomes as assigned.
  • Solid writing and verbal communication. Comfortable making presentations to large and small groups.
  • Requires computer proficiency including ability to learn new data systems and manage work across multiple systems, word processing skills.
  • Must have and maintain a valid driver’s license that meets The Salvation Army insurance requirements.
  • Must have regular and consistent access to a reliable, registered, and insured vehicle for travel across the Division.
  • Perform all duties associated with job responsibilities and tasks as assigned.
