Job Description: Under the direction of the Manager and as part of an interdisciplinary team, the Assessor Coordinator - Degree will provide case management and coordination services to a vulnerable sector of the community. The incumbent will provide consultation and coordination service including prioritizing and referring to community agencies relating to the determinant of health needs. The Assessor Coordinator - Degree, along with the community and the Urgent Care teams, provides crisis intervention, support, assessment and follow-up plans to clients in the community who require assistance. The position will provide intervention and support by referring to SHA PHC and/or community programs. Follow up may include brief intervention/support, referral to appropriate community organizations/support networks or collaboration with existing agencies. Connection to both SHA and community support organizations is the goal.

Education

  • Bachelor Degree in a related Health Science or Social Science discipline

Competencies

  • Basic - Computer skills

Knowledge and Abilities

  • Knowledge of assessment, triage, treatment and case management.
  • Ability to develop & maintain consulting relationships in mutidisciplinary team
  • Demonstrates adherence to established standards of care/ SHA Policies/Procedures
  • Ability to refer to, network, and work cooperatively with community agencies
  • Advocates delivery of culturally safe, responsive & people-centered healthcare
  • Sensitive to diverse cultures/client advocay & social justice concerns.
  • Ability to formulate evidence-based intervention based on clinical presentation
  • knowledge of supports which are strengths based and culturally responsive
  • Highly developed written and oral communication skills
  • Ability to enter and retrieve data in computer database systems
  • Adjusts clinical approaches in a culturally responsive and respectful manner
  • Knowledge of the Truth and Reconciliation Commission Calls to Action
  • Knowledge of community resources
  • Strong organizational skills with ability to prioritize urgent client needs
  • Demonstrates critical thinking, problem solving, conflict resolution skills
  • Knowledge of Occupational Health and Safety Standards and application of such
