Job Description: Under the direction of the Manager, the Assessor Coordinator is responsible as the primary coordinator of client admissions at the Urgent Care Centre (UCC). The incumbent of this position is an integral member of the multidisciplinary team. The incumbent is responsible for review of community Mental Health & Addictions assessments, screening, consultation, date collection, organizing and communicating client admissions, and client family and community education. The incumbent will adhere to the current Provincial Clinical Addiction Model for Practice and SHA philosophy, objectives and standards. They would be familiar with current drugs of abuse, concurrent disorders and withdrawal management strategies. The incumbent will work with UCC staff and in collaboration with Manager regarding program development to ensure continuous improvement occurs using evidence-based information. The incumbent is responsible for providing case consultation, case assignment and orientation.
