Job Description: Under the direction of the Manager, the Assessor Coordinator is responsible as the primary coordinator of client admissions at the Urgent Care Centre (UCC). The incumbent of this position is an integral member of the multidisciplinary team. The incumbent is responsible for review of community Mental Health & Addictions assessments, screening, consultation, date collection, organizing and communicating client admissions, and client family and community education. The incumbent will adhere to the current Provincial Clinical Addiction Model for Practice and SHA philosophy, objectives and standards. They would be familiar with current drugs of abuse, concurrent disorders and withdrawal management strategies. The incumbent will work with UCC staff and in collaboration with Manager regarding program development to ensure continuous improvement occurs using evidence-based information. The incumbent is responsible for providing case consultation, case assignment and orientation.

Education

• Bachelor Degree in a related Health Science or Social Science discipline
• Bachelor degree in Social Work, Psychology or other health related discipline

Knowledge and Abilities

• Demonstrated skills with Mental Health & Addiction specific assessment and counselling.
• Knowledge of community Mental Health & Addiction assessment referrals and applicable programs.
• Demonstrated knowledge of both youth and adult Mental Health & Addiction and treatments.
• Demonstrated knowledge of appropriate models (Trans theoretical Model of Change, Motivational Interviewing)
• Proficient computer skills with particular emphasis on work processing, excel and spreadsheets.
• Demonstrated high degree of accuracy is required in all aspects of assigned responsibilities.
• Trained in current ASIST (Applied Suicide Intervention Skills Training) suicide risk assessment model.
• Applied knowledge of drugs of abuse and their effect both during use and during acute and post-acute withdrawal.
• Applied knowledge of Suboxone/Methadone/Sublocade assisted recovery programs.
• Applied knowledge of harm reduction principles in Addictions Medicine.
• Demonstrated knowledge in the area of assessment and referral coordination.
• Knowledge of how addiction and mental health impacts the individual, community and families.
• Ability to accurately review assessment findings.
• Ability to make decisions in accordance with the policies and procedures of SHA Mental Health and Addictions Services and support other employees in also following these guidelines.
• Ability to plan, organize and coordinate efforts with other multidisciplinary team members as well as communicate effectively and adapt to changing conditions.
• Demonstrate resourcefulness, ability to work independently, and in collaboration with others.
• Knowledge of provincial human service organizations, resources and referral procedures.
• Superior communication, organizational and interpersonal skills.
• Sound knowledge of the Mental Health Services Act and Health Information Protection Act
• Initiative in organizing work in accordance with departmental procedures and in completing assigned tasks.
• Demonstrated ability to work under pressure, meet deadlines and manage time and stress
• Good attendance required.
