Salary or Pay Band: Pay Band Assessor Coordinator - Degree $36.721 to $44.902

The overall function of the assessor/coordinator (client care coordinator) position is to assess and coordinate single entry access to long term care, respite and community day programs. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Human Resources Exemption: No

Education

  • Bachelor of Science in Nursing (BScN)

Other Education and Training

  • Bachelor of Social Work is acceptable

Knowledge and Abilities

  • Knowledge in the SHA, community resources and applicable programming
