Salary or Pay Band: Pay Band Assessor Coordinator - Degree $36.721 to $44.902

Travel Required: Yes

This position reports to the Manager or his/her designate and is responsible for the assessment and coordination of Home Care Client services and case management. The assessment process forms the basis for all activities performed by Home Care, as well as referrals to other appropriate agencies and long-term placement. The position responsibilities and functions are fulfilled with an underlying philosophy of engaging the client and family members, encouraging client empowerment and self- efficacy. Client empowerment is achieved with self-care models, and utilizing Client Centered Care Planning with client goals and outcomes. Confidentiality and HIPA guidelines are maintained at all times. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement."

Education

  • Bachelor Degree in a related Health Science or Social Science discipline

Licenses

  • Valid Class 5 driver's license

Other Education and Training

  • Degree in a Health or Human Services field acceptable to the Employer

Knowledge and Abilities

  • Ability to work independently and as a member of a team
  • Excellent public relations skills
  • Ability to apply advanced computer skills
  • Has demonstrated strong critical thinking and analytical skills
  • Knowledge of assessment processes and terminology.
  • Ability to work well with different types of personalities in various settings.
  • Demonstrates effective verbal and written communication skills.
  • Knowledge of Assessment and Development care plans.
