
As a Case Manager II at a permanent supportive housing, your role will involve providing comprehensive support services to individuals and families experiencing homelessness. You will be responsible for assessing participants' needs, developing individualized service plans, and connecting them with appropriate resources to help them regain stability and move towards permanent housing. Your primary objective will be to empower clients to overcome barriers and achieve self-sufficiency.
