At Concord Hospital Medical Group (CHMG), our Advanced Practice Registered Nurses (APRNs) and Physician Assistants (PA-C) are essential members of a dynamic, patient-centered primary care team. In this role, you’ll deliver comprehensive preventive and primary medical services to patients across the lifespan, building meaningful relationships while providing high-quality, evidence-based care.
Practice exclusively in a supportive outpatient setting alongside experienced physicians, APPs, clinical teams who value collaboration and teamwork. You’ll have the resources and clinical support needed to focus on what matters most: your patients.

- APRN: Master’s or Doctoral degree from an accredited Nurse Practitioner program.
- PA‑C (Physician Associate): Master’s degree from an ARC‑PA–accredited Physician Assistant program.
Licensure, Certification, Registration
- Unencumbered New Hampshire APRN or PA-C license.
- National certification in relevant specialty (APRN) or NCCPA (PA-C).
- Eligible for or current DEA license.
Experience
Prior primary care experience required.
Responsibilities
- Partner with patients to understand their health history, perform comprehensive examinations, and develop thoughtful, evidence-based diagnoses.
- Use diagnostic tools effectively to guide care and ensure timely follow-up.
- Manage a wide range of acute and chronic conditions with clinical expertise and judgment.
- Prescribe and monitor medications safely, ensuring optimal patient outcomes.
- Educate patients on preventive health and wellness to empower informed decisions.
- Maintain productivity standards while delivering high-quality, patient-focused care.
- Document care thoroughly and accurately, supporting continuity and quality of care.
