Job Description

Job Location: Center For Human Services - Stratford, CT 06615SUMMARY:
In accordance with RNP's Mission Statement, Philosophy of Client Care and Code of Ethics as stated in the Personnel Policies and under the supervision of
SPECIFIC DUTIES AND RESPONSIBILITIES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA).
- Conduct comprehensive medical evaluations for individuals receiving outpatient SUD treatment
- Perform MOUD inductions and manage ongoing pharmacotherapy (methadone, buprenorphine, naltrexone as applicable)
- Implement and document dose adjustments based on clinical assessment, withdrawal symptoms, labs, and treatment response
- Assess and manage withdrawal symptoms and co-occurring medical or psychiatric conditions
- Review laboratory results, toxicology screens, EKGs (when indicated), and other diagnostics
Correctional Health Services
- Provide clinical services within Department of Corrections settings, adhering to all security and operational protocols
- Collaborate with both RNP and DOC nursing staff, custody staff, and administration to ensure continuity and safety of care
- Support continuity of care for individuals transitioning between incarceration and community-based treatment
Interdisciplinary Collaboration
- Participate in treatment team meetings, case conferences, and utilization reviews
- Collaborate with nursing, counseling, recovery support, and program leadership
- Provide clinical consultation and guidance to nursing staff related to MOUD and medication safety
Documentation & Compliance
- Complete timely, accurate documentation in the electronic health record
- Ensure compliance with CT DPH, DMHAS, DEA, DOC, ASAM, and CARF standards
- Maintain accurate records related to inductions, dose changes, progress notes, and medical decision-making
- Participate in audits, quality improvement initiatives, and survey preparation activities
Hours: Per Diem
QUALIFICATIONS:
- Current Connecticut APRN or PA license in good standing
- DEA registration (or eligibility to obtain)
- Experience or demonstrated competency in MOUD
- Knowledge of evidence-based substance use disorder treatment and ASAM Criteria
- Strong clinical judgment, documentation skills, and ability to work collaboratively
- Ability to function effectively in outpatient and correctional environments
