Provide inpatient palliative care consultations and follow-up care for patients with serious illness across the AHN Palliative Care Program.
Manage initial and follow-up palliative care consultations, prioritizing patients by acuity and collaborating withadynamicInterdisciplinary team
Support complex symptom management, serious illness communication, goals-of-care discussions, hospice evaluations,and end-of-life care planning.

Scope of Practice / WhatYou’llDo
Provide initial and follow-up palliative care consultationsforinpatient patients.
Partner with physicians for collaboration while functioningat the top oftheAPP scope of practice.
Support patients and families through symptom management, hospice evaluations, goals-of-care conversations,and end-of-life planning.
Participate in interdisciplinary collaboration with physicians, APPs, hospice liaisonsupportand palliative counselors.
Procedures are notrequiredfor this position.

Qualifications
Nurse Practitioner withappropriate Georgialicensure required or ability to obtain prior to start.
Acute Care NP, Primary Care NP certificationrequired, as applicable to licensure and role scope.
New graduates will be considered.
2 to 3 years of Hospice and/or Palliative Care experience, or related experience caring for patients with serious illness.
Hospice and Palliative Medicine (HPM) certification preferred.
Strong skills in symptom management, serious illnesscommunicationand end-of-life care
