We are seeking a skilled and compassionate Ambulatory Care Pharmacy Technician to join our dynamic team. The Ambulatory Care Pharmacy Technician will work at Transition of Care/Skilled Nursing Facilities (TOC/SNF) for the equivalent of 5 days a week (1 FTE). This role supports our ambulatory care services by assisting pharmacists and the TOC/SNF team in medication management, patient education, and ensuring optimal medication therapy outcomes.
Essential Functions
Clinical Support:Support pharmacists and TOC/SNF teams in conducting Collaborative Drug Therapy Management (CDTM) and patient assessments. Document patient interactions and medication therapy outcomes, including but not limited to:-Gathering medication histories with accuracy from multiple Electronic Health Records (EHR).-Identifying potential errors or necessary clarifications with acute precision.-Medication use and administration.-Prescribing accuracy.-Administration of CDTM per protocols.
Administrative Duties:Assist with insurance processing and billing as needed, including but not limited to:-Alleviating pharmacists' and TOC/SNF staff’s time by completing various administrative tasks.-Delivering safe patient care.
Patient Interaction:Address patient questions and concerns, and facilitate medication access.
Compliance:Ensure compliance with all relevant regulations and standards, including those set by the Board of Pharmacy and other regulatory agencies.
MINIMUM EDUCATION High School Diploma or equivalent (higher degree accepted)
MINIMUM EXPERIENCE 1 year of experience as a pharmacy technician
REQUIRED LICENSURE/CERTIFICATIONS
-Current Basic Life Support (BLS) - Obained through approved American Heart Association (Upon Hire)
-Certified Pharmacy Technician license with Texas State Board of Pharmacy (Upon Hire)
-Completion of Medication Therapy Management (MTM) and Medication History Certificate through PTCB.org (Within 6 months of hire)
-Medication History Certificate (within 6 months of hire)
