Ambulatory Pharmacy Services provides comprehensive outpatient pharmacy care to patients before, during, and after their healthcare visits. The team works closely with providers to ensure safe and effective medication therapy through prescription dispensing, medication counseling, medication therapy management, prior authorizationassistance, and patient education. Ambulatory pharmacists help improve medication adherence,optimizetreatment outcomes, and support patients in managing chronic and acute health conditions while promoting continuity of care across the healthcare continuum.
Partnerswith providers and interdisciplinary care teams to integrate clinical pharmacy services andoptimizemedication therapy for ambulatory patients.
Performs comprehensive medication reviews, medication reconciliation, disease state management, pharmacokinetic monitoring, and evidence-based medication therapy recommendations.
Provides patient education, medication adherence support, refill coordination, financialassistanceresources, and ongoing clinical follow-up to improve outcomes.
Documents clinical interventions in the electronic medical record whilemaintainingcompliance with regulatory, accreditation, and organizational standards.
Participates in multidisciplinary committees, quality improvement initiatives, and educational activities for healthcare professionals, patients, and families.
Supports the advancement of pharmacy practice through professional development, community outreach, and preceptingpharmacystudents and residents as applicable.
Requirements:
Bachelor's degree; graduate of an accredited College of Pharmacy
Minimum one year of pharmacist experience
KentuckyBoardof Pharmacy license in good standing
