We are building an innovative, team based clinic designed to deliver comprehensive, same day care for patients with complex chronic conditions, and we are looking for skilled, proactive nurses to help us launch.
In this role, you will balance hands on patient care with meaningful clinical coordination. You will conduct nurse visits where you review care protocols, educate patients, and administer injections or simple procedures. You will also support desk based clinical work including telehealth visits, telephonic nursing, pre visit planning, and chart preparation and review.
You will work primarily with patients managing chronic conditions such as:
• Irritable Bowel Disease (IBD)
• Interstitial Lung Disease (ILD)
Our model centers on real time, collaborative care. Patients are seen by the full care team in one coordinated appointment. We are seeking individuals who advocate for their patients, think proactively, and confidently step in to support both patients and colleagues.
