Position Summary:

The Ambulatory Care Pharmacist plays a key role in advancing this mission by delivering advanced, patient-centered clinical pharmacy services within a Federally Qualified Health Center (FQHC) setting.
This position focuses on comprehensive medication management, chronic disease management, and population health initiatives to improve clinical outcomes and expand access to high-quality, cost-effective care. The Ambulatory Care Pharmacist collaborates closely with providers and interdisciplinary teams to optimize medication use, support value-based care, and address the needs of diverse and underserved patient populations. The role also contributes to grant-supported programs, quality reporting, and the ongoing development of pharmacy services across the organization.
Primary Responsibilities:
- Evaluate drug therapy through direct patient care involvement, with clinical assessment, subjective and objective findings relating to patients’ responses to drug therapy and communicating and documenting those findings and recommendations to appropriate individuals and in appropriate records (i.e., patient’s medical record).
- Performs patient assessments and monitors the effects of drug therapy using clinical symptoms, evaluating laboratory data, and other relevant clinical diagnostic techniques.
- Interviews patients to obtain information regarding drug use, drug allergies and sensitivities, and medication histories; assesses compliance and counsels patients on appropriate drug use.
- Under protocol, initiates, adjusts, monitors, and recommends changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, review of drug profiles and the patient medical record.
- Formulate patient treatment plans and initiate appropriate drug therapy as outlined in approved treatment protocols or in consultation with the primary physician.
- Initiate requests for appropriate laboratory and other diagnostic studies as needed for monitoring drug therapy.
- Analyze laboratory and diagnostic test data to modify drug therapy and dosing as necessary
- Appropriately refer primary care patients to other health care practitioners, including nutritionists, podiatry, optometry, nurses, social workers, and others in collaboration with primary care physician or advanced practice provider.
- Provide pharmacologic consultations to physicians, nurses, pharmacists, students, and patients.
- Provide patient education regarding drug therapy in both formal and informal forums.
- Coordinates and interprets laboratory monitoring of drug therapy, including serum drug levels, electrolytes, and other approved drug-related laboratory testing.
- Provides patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization and patient compliance, detecting and/or minimizing ADR's, eliminating unnecessary drug use and duplication.
- Provides patient education related to the individual's disease state, including predictable actions, side effects, reasons for proper compliance, and any special instruction.
- Deliver MTM services, including but not limited to, Comprehensive Medication Reviews, Disease Specific Medication Reviews, and Targeted Interventions to assess appropriateness of all medications, to optimize medication therapy and to improve clinical outcomes
- Evaluate and document clinical responses to therapy including prior adverse reactions and efficacy
- Collaborate with prescribers, interdisciplinary care teams, patients and/or caregivers to identify and develop patient-specific disease state therapy goals
- Under protocol, respond to refill authorization requests and assist with approving alternative medication requests and prior authorizations
- Support pharmacy operations as needed
- Verify prescriptions and assist workflow
- Provide immunization
Population Health & FQHC Initiatives:
- Support UDS measures and quality metrics
- Manage high-risk patients
- Participate in care gap closure initiatives
- Support transitions of care
- Optimize therapy using 340B formulary
- Ensure compliance with 340B requirements
- Support cost-effective prescribing
- Support grant initiatives and reporting
- Assist with data analysis and outcomes tracking
- Contribute to HRSA and UDS reporting

QualificationsRequired
- Doctor of Pharmacy (PharmD) from an accredited institution
- Active and unrestricted California Pharmacist license
- Completion of a PGY1 Pharmacy Residency
Preferred
- PGY2 Residency
- Board Certification (e.g., BCACP, BCGP)
- Advanced Practice Pharmacist (APh) certification
- Medication Therapy Management (MTM) certification
- Bilingual Spanish preferred
- Knowledge and experience with the 340B program
