
Location: 100% Remote | Active Massachusetts Pharmacist License Required
This is a fully remote position. Candidates must hold an active, unrestricted pharmacist license from the Massachusetts Board of Registration in Pharmacy at the time of hire, and keep it in good standing for as long as employed.
The Ambulatory Care Clinical Pharmacist is responsible for the promotion of rational, safe and cost-effective drug therapy in collaboration with both internal and external primary care providers & specialists, nurse care managers, our community health partners and other non-clinical roles. Identify and help resolve medication therapy opportunities while co-designing individualized therapeutic regimens for optimal management of chronic disease states and preventative care. Provide direct care to members and their caregivers to deliver Comprehensive Medication Management (CMM) and help drive better individual and population health outcomes while preventing avoidable Emergency Department visits and hospitalizations.

This role is eligible for health insurance, life insurance, retirement benefits, participation in the company’s equity program, paid time off, including vacation and sick leave. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location. The expected salary range for this position is:
150,000 - 162,000
