ESSENTIAL DUTIES AND RESPONSIBILITIES including the following:
- . Have a minimum of two monthly case management session with each inmate/client.
- Maintain Case Management progress notes on all inmates/clients.
- Be available to make referrals for emergency counseling when needed.
- Assist client to become gainfully employed. Provide employability services.
- Develop a discharge summary and a discharge profile on each inmate/client who leaves the program.
- Works directly with the inmates and local service providers to plan for reintegration from incarceration into the community.
- Conducts initial pre-release screening to determine the level of service and support needed.
- Provides compliance of case management services within the facility.
- Ensures individual participant case planning encompasses key partners, support and services.
- Completes assigned trainings, including the Florida Department of Corrections 100 hour Transition Program Facilitator Training.
- Facilitates assigned classes and programs, including Florida Department of Corrections 100 hour Transition Program.
- Maintains relationships & contracts within network of local service providers.
- Attend all staff meetings.
- Visit each inmate’s/client’s place of employment and maintain records of visits.
- Document phone calls to employers.
- Maintain up to a maximum of 50 inmates/clients on Case Management load.
- Follow all applicable policies and procedures.
- Be available to work all shifts to ensure proper coverage per contract and policy standards.
- Clock in and out utilizing the company time and attendance system

- Perform other related duties as assigned by immediate supervisor.
QualificationsEXPERIENCE: Three (3) years’ experience in Case Management preferred. One (1) year substance abuse experience preferred. Must be proficient in Microsoft programs. Must be flexible and adept with multi-tasks. Must be a self-starter and able to work independently. Must be able to communicate effectively. Must be result driven.
EDUCATION: High School Diploma. Associates’ Degree in any Social Science field preferred.
