
Springpoint At Home is seeking an experienced Part-Time Aging Life Care Advisor (TM) to join our team!
Springpoint At Home offers home care and care management services to residents of our Life Plan Communities as well as to individuals that live in their private home. At Springpoint, our Aging Life Care Advisor (TM) is a health and human services professional who acts as a guide and advocate for their clients and/or families. The Aging Life Care Advisor (TM) will maintain a specialized focus on issues related to aging and elder care and is responsible for daily Care Management and activities.

Professional and Organizational Responsibilities
Education and Experience:
Graduate degree in Social Work, Gerontology, therapy or related field
Minimum of 3–5 years experience in geriatric Care Management or in Psychotherapy required
Experience in Heal Care Sales/Marketing required
Experience in conflict mediation & family therapy
Certification in Care Management preferred
Membership in the Aging Life Care Association® required
Comprehensive Benefits Package (Part Time):
Pay Range: $45 - $47 / hr
