Examples of duties
This position is responsible for evaluating the needs of older adults, developing plans, and coordinating services to provide clients with the most effective possible care and support. As an Aging Care Manager 2, you will perform the following duties:
- Intake and Assessment: Conduct comprehensive assessments to evaluate each person’s level of need for facility, agency, or referral services; problems related to independent living; and social, emotional, and other capabilities
- Care Plan Development: Include all available assessment results and information to recommend a comprehensive plan that helps maintain the consumer’s independence for as long as feasible
- Consumer Support: Meet with individuals, their families, and hospital officials at their home, a hospital, or other care facility; explain agency services; and help older adults and families complete forms and connect with services
- Ongoing Care Management: Make referrals to appropriate community services, coordinate service delivery, and provide on-going assessment, re-evaluation, and follow-up
- Documentation: Complete all paperwork and enter confidential client information into state databases within required deadlines
- Professional Collaboration: Regularly coordinate services with medical teams, community agencies, financial institutions, care homes, and other resources; participate in outreach efforts; and respond to client emergencies and protective services
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch, and periodic on-call hours when required after training. Some training would be out of county and may require overnight stay.
- Telework: You will not have the option to telework in this position.
- Salary:In some cases, the starting salary may be non-negotiable.

Qualifications
QUALIFICATIONS
Minimum Experience and Training Requirements:
- Six months as an Aging Care Manager 1 or a County Caseworker 1; or
- Successful completion of the County Social Casework Intern program; or
- Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.

- Hiring preference for this vacancy may be given to candidates who live within Schuylkill County. If no eligible candidates who live within Schuylkill County apply for this position, candidates who reside in other counties may be considered.
