Aged Care On-Site Pharmacist

Grade 3 (Yr 1, 2 or 3)

0.4 FTE, 32 hrs per f/n

Medical Scientists, Pharmacists and Psychologists Victorian Public Sector (Single Interest Employers) Enterprise Agreement 2021-2025

YDMH is excited to announce we are seeking an On-Site Aged Care Pharmacist for a Fixed Term role from 19/10/2026 to 19/10/2027.

This role will work collaboratively with residents, families, nursing staff and medical practitioners to improve health outcomes of residents, supporting safe effective care.

Key Responsibilities:

  • Person centred care – collaborating and communicating effectively with residents, family and carers to understand resident preferences, beliefs and priorities.
  • Collaboration and communication – develop an interprofessional working relations, integrating with the multidisciplinary teams, both internally and externally.
  • Medication Management – Provide expertise in medication management, reducing medication related harm and optimisation of medications.
  • Quality Improvement – facilitate quality improvement activities, including analysing, reporting of results and recommendations.
  • Clinical Governance – this role will contribute to RACF clinical governance and encourage a culture of continuous quality improvement, ensuring safe and quality care for residents.

We are the major specialist paediatric hospital in Victoria and our care extends to children from Tasmania, southern New South Wales and other states around Australia and overseas.

With a passionate, highly skilled and committed staff campus wide of over 5,000, we provide a full range of clinical services, tertiary care and health promotion and prevention programs for children and young people.

We are the designated state-wide major trauma centre for paediatrics in Victoria and a Nationally Funded Centre for cardiac and liver transplantation.
