The RN II Advanced Primary Care role provides virtual care to patients that benefit from ongoing chronic disease management. In partnership with a multidisciplinary team, including physicians and advance practitioners, the RN is responsible for creating patient relationships, monitoring vital sign trends, conducting patient visits in between regular office visits, assisting in panel management of patients and providing individualized care to improve patient outcomes.  Reports to respective Advanced Primary Care Lead APP.

What You’ll Do:

As an RN II in Advanced Primary Care, you’ll work closely with a multidisciplinary team—including physicians and advanced practitioners—to provide ongoing support to patients with chronic conditions. Your responsibilities will include:

  • Building strong patient relationships and monitoring vital sign trends.

  • Conducting virtual visits between regular office appointments.

  • Supporting chronic disease management, discharge follow-up, and acute care needs.

  • Advancing guideline-directed medical therapy (GDMT) and facilitating advance care planning for Medicare and MA patients.

  • Collaborating with primary care clinicians in ambulatory settings to coordinate care.

  • Performing virtual assessments, patient education, triage, and escalation of care needs.

  • Assessing medically complex patients and acute exacerbations of chronic conditions.

  • Coordinating care with a multidisciplinary team including specialists, social workers, physical therapists, and palliative care providers.

  • Documenting all care provided and maintaining patient confidentiality.

  • Practicing within the RN scope under the supervision of an advanced practitioner.

What You’ll Bring:

  • Active RN license in applicable state(s).

  • Experience in chronic disease management and virtual care preferred.

  • Strong communication and collaboration skills.

  • Commitment to Mercy’s Nursing Conceptual Framework and patient-centered care.
