Job Description

Reports to: Chief Medical Officer, Chief Clinical Officer/Director of APP Residency Program

Salary/Status: Flat stipend with standard employer contribution of benefits

Minimum Requirements:
1. Anticipated graduation from an MSN/DNP Family Nurse Practitioner program OR Master’s level Physician Associate program OR less than one year of clinical practice post graduation from a higher education program.
2. Passing of ANCC or AANP certification exam) OR completion/passing of NCCPA board exam prior to the anticipated start date (rolling; typical target date is May 31)
3. Ability to attain professional practice, Mass CSR, and DEA licenses prior to the yearly start date.
4. Ability to collaborate with multi-disciplinary teams, including physicians, nursing staff,
social workers, and case managers.
5. Previous experience in ambulatory care and/or public health is preferred.

Desirable:
1. Fluency in Spanish, Arabic, Russian, Nepali, Vietnamese or another language served at CHC.
2. Special expertise in the provision of health care to underserved, culturally diverse populations.

Principle Responsibilities and Duties:
1. Participates as a student in didactic classes as scheduled (weekly but subject to change) and presents patient cases as assigned.
2. Complete weekly journal entries to track personal and professional development through the program.
3. Adequately complete proposed quarterly non-clinical assignments including quality improvement tasks, community service events, journal article review and precepting of NP/PA students.
4. Conduct comprehensive patient assessments.
5. Identifies potential and actual health and health-related problems
6. Evaluate patient responses associated with the problem area(s).
7. Diagnoses acute and chronic illness.
8. Develop and implement treatment plans that include pharmacologic and non-pharmacologic components.

9. Includes the patient and family as active participants in the treatment plan.

10. Focuses the provider/patient interaction on wellness, health promotion, disease prevention, and self-care.
11. Offers comprehensive clinical family planning services when appropriate.
12. Review patient charts and ensure appropriate treatment plans are developed and followed.
13. Completes all charts on the same day of care or no later than 72 hours after the visit.
14. Patient charts are completed in a manner that meets all risk management and chart audit requirements.
15. Collaborates and consults with CHC physicians and other providers. When deemed necessary refer patients to appropriate specialists and other practitioners and facilities.

16. Cooperates with efforts of CHC administration to monitor and improve the quality of patient care by evaluating patient outcomes.
17. Maintains updated Nurse Practitioner/ Physician Associate guidelines.
18. Participate in peer review of charts.
19. Maintains all CME/CEU, Board, and Licensing requirements of state and national governing Boards.
20. Duties as assigned

This position requires compliance with Caring Health Center’s compliance standards including its Standard of Conduct, Compliance program, and policies and procedures. Such compliance will be an element considered as part of the Nurse Practitioner’s regular performance evaluation.

This position also requires the ability to use a computer, work at a computer workstation, view a CRT, and have some knowledge of a keyboard.
