Who We Serve
Our Atlanta-based clinic in the Old Fourth Ward proudly serves a diverse, vibrant, and inclusive patient population. We’re looking for an APP (Nurse Practitioner or PA) who values cultural competence, open-mindedness, and genuine human connection. At Ascend, we don’t just treat conditions—we honor the whole person.

We are designing our care model to support patients on a journey towards longevity and happiness. We currently offer a hybrid delivery model of virtual and at home meets patients where they are: at home, at work or anywhere else with comprehensive Virtual and Mobile Care. We are partnering with traditional PCPs to augment their offering, allowing patients to experience Ascend through physical, digital and at-home channels. Furthermore, Ascend accepts all major insurance plans.

Requirements
Requirements- MUST BE ABLE TO WORK IN THE ATLANTA, GA OFFICE (Old Fourth Ward Neighborhood)
· Master’s degree from an AMA accredited school for Advanced Practice Providers.
· Current, active PA or NP licensure in Georgia is required.
· Internal Medicine or Family Medicine Board Certification (or board eligible)
· Active, non-probationary, unrestricted Georgia State License
· Active DEA license or eligibly to obtain
· 3+ years clinical experience in internal medicine or primary care practice setting.
· Technological proficiency—comfortable using EMRs and digital health tools to enhance care delivery
