About Us:Western Illinois Home Provider Care, an affiliate of Western Illinois Home Health Care, is a primary care practice focused on building strong, lasting relationships with our patients by delivering high-touch, cost-effective, convenient care. Our model allows us to prioritize time with each patient, providing comprehensive care that focuses on prevention, wellness, and long-term health goals. As an Advanced Practice Provider for our agency, you’ll have the autonomy to provide full-spectrum primary care in a low-stress, high-impact environment. You’ll work closely with patients to manage their health, promote wellness, and treat acute and chronic conditions – without the high patient volumes or burdensome bureaucracy. We offer our patients house calls just like medicine used to be.
