Job Description

Central Ohio Primary Care is seeking a full-time Advanced Practice Provider (NP/PA-C) for Tri-County Family Physicians, 11925 Lithopolis Road, NW, Canal Winchester, OH 43110.

The APP, in collaboration with the Physicians, coordinates the healthcare of primary care patients at this location. This position monitors the patient’s status and collects data to evaluate and improve care by providing a comprehensive assessment, implementing the appropriate treatment, and evaluating the patient’s progress.

About the area and practice:

Canal Winchester is located fifteen miles southeast of Columbus. It has a small-town ambiance with the convenience of shopping, restaurants, parks, festivals, and historical museums and a twenty-minute drive to Columbus.

This COPC practice has established a reputation as a primary care group that serves the community. They provide the best primary care for all ages with a focus on health education, disease prevention and management to keep their patients well.

It is a large outpatient practice consisting of (6) family physicians and 2 APPs. There are several medical assistants and patient service reps to support the providers and patients. This opening is due to a provider retiring this summer.

Tri-County Family Physicians is open 6 days a week: 8-5:00 pm M-F and 8-12:00 pm on Saturday. Your work schedule can be adjusted as needed.

Key Responsibilities:

Conduct thorough assessments, including medical history reviews and physical examinations, to diagnose and treat acute and chronic health conditions in patients of all ages.

Provide a full spectrum of primary care services, including well visits, health promotion, disease prevention, routine screenings, vaccinations, and management of common illnesses and injuries.

Develop individualized treatment plans for patients with chronic diseases such as diabetes, hypertension, asthma, and COPD. Monitor disease progression, adjust treatment regimens, medication management and educate patients on self-management strategies.

Empower patients and families by providing comprehensive education on health maintenance, disease prevention, and treatment options. Address questions, concerns, and health-related inquiries to promote informed decision-making and adherence to care plans.

Collaborate with physicians, advanced practice providers, nurses to coordinate patient care across multiple disciplines and healthcare settings. Facilitate referrals to specialists and other healthcare providers as needed.

Maintain accurate and timely documentation of patient encounters, treatment plans, and follow-up care using electronic health record systems in compliance with regulatory standards and organizational policies. COPC uses EPIC.

Qualifications:

Master’s degree in nursing or physician studies from an accredited program.

Current licensure as a Nurse Practitioner or Physician Assistant in state of Ohio.

National certification as a Family Nurse Practitioner (FNP) or Physician Assistant (PA).

A minimum of 1 year of practical working experience as a NP/PA post graduation in an outpatient practice setting.

Strong clinical assessment, diagnostic, and treatment skills.

Excellent communication and interpersonal skills.

Ability to work independently and collaboratively within a multidisciplinary team.

EPIC EHR experience is helpful. Training is available at onboarding.
