Key Responsibilities:
Conduct comprehensivesurvivorshipassessments including history, physical examinations,andinterpretation of diagnostic studies.
Develop,implementand regularly update individualized survivorship care plans following completion of cancer treatment.
Summarize treatment history (including surgery, chemotherapy, radiation, andimmunotherapy) andoutline follow-up care
Coordinate transition from active oncology care to long-term survivorship.
Monitor for cancer recurrence and secondary malignancies
Oder and interpretappropriate surveillancelabs, imaging, and diagnostic tests.
Identifyand managelate and long-term effects ofcancer and its treatment, including referral to rehabilitation, specialty services, or supportive therapies as needed.
Screen patients for psychosocial or emotional impacts and provide counseling and referrals as needed.
Counsel survivorson health promotion, disease prevention, and age/riskappropriate screeningsand vaccinations
Provide patient andcaregivereducation.
Maintain compliance with organizational policies, regulatory guidelines, and best practice standards.
Qualifications:
Graduate of an accredited Physician Assistant or Nurse Practitioner Program
National Advanced Practice Registered Nurse Certification (APRN) OR national board certification as a Physician Assistant.
Current Physician Assistant or APRN license in the State of Florida.
AOCNP Certification
BLS/ACLS
Preferred Skills:
2 years of Oncology Experience
Experience with Epic Electronic Medical Record
