What you can expect at your practice:

• Hiring a Certified Registered Nurse Practitioner (CRNP) – Adult‑Gerontology Nurse Practitioner (AGNP) or Family Nurse Practitioner (FNP)
• Monday–Friday schedule with flexible hours to support work–life balance
• Shared call rotation with the provider team:
  • One weeknight of call per week
  • Every sixth weekend
• Opportunity to support high‑quality patient care at Pleasant Acres Nursing & Rehabilitation Center
• Joining an established, collaborative practice consisting of:
  • Two physicians
  • Three Nurse Practitioners
• Ideal for Nurse Practitioners seeking a rewarding role in long‑term care, rehabilitation medicine, and complex adult care
• Provide comprehensive primary care, chronic disease management, and patient-centered services for adult and geriatric populations
