Dartmouth Health is seeking an experienced Nurse Practitioner or Physician Assistant to join our Geriatric Medicine team to provide primary care services in Lebanon, New Hampshire. The newly formed Geriatric Medicine Section formally nested under Internal Medicine, led by Section Chief, Dr. Serena Chao and VP of the Geriatric Centers of Excellence Dr. Ellen Flaherty, is an interdisciplinary team with support providers including: care coordination, dietitian consults, pharmacy services, and embedded psychiatry services.
Your involvement in working within a team is an important aspect of this position but also with the ability to work autonomously. Prior advanced practice experience is desired which should include primary care and preferably geriatrics settings as well.
