About this role:
Intermountain Healthis seeking an experienced Nurse Practitioner or Physician Assistant for our Heart Failure Clinic.The selected candidatewill partnerwith thetwoboardcertifiedHeart Failure Cardiologists tocontinue tobuildapre-eminent Heart Failure Program in the state of Colorado.
You would practice at our Denver Cardiology clinic and Saint Joseph Hospital Additional locations may be added with program growth
Hours are 4/10 hour shifts during the week, nocalland no weekends
You would workwith stage 3 and 4 heart failure patients to ensure medication compliance,monitoringdietary and lifestyle changes, regular follow ups and much more
This position would be part of the continuity of patient care from hospital to clinic and back to the primary care physician or cardiology service line
You would worka combination of outpatient clinicconsults and follow ups along with inpatient roundingand consults

Whatyou’llbring:

Graduate from an accredited Physician assistant or Nurse Practitioner program
Licensed as an NP or PA in the state of CO or able to obtain
DEA and full prescriptive authority in COor able to obtain
BLSand ACLSrequired
Minimum of 2 yearsof experience as an NP or PA in a cardiology setting (PLEASE NO newgrads)
Experience in aHeart Failure program, highly desired
