Salud Para La Gente (Salud) provides high quality, comprehensive and cost-effective full scope primary care services to underserved low-income communities in the Monterey Bay area, including Santa Cruz County and Monterey Counties. Salud began in Watsonville in 1978 as a storefront “free clinic,” and with a growing need for health services, in 1992 became a Federally Qualified Health Center (FQHC). Today Salud is a primary health care network with 11 clinic sites and over 500 employees. We provide high quality services to patients of all ages. We are looking for an experienced, enthusiastic, compassionate Advanced Practice Clinicians (Nurse Practitioner or Physician Assistant) who will work with the medical team to fulfill our mission to provide high quality, comprehensive and cost-effective healthcare responsive to the needs of the community we serve.

Under the direction and supervision of the Director of Advanced Practice Clinicians, the Advanced Practice Clinician provides basic medical care to clients, in accordance with established protocols. Care includes diagnosing, treating, and providing preventive medical services to primarily low-income Spanish-speaking farmworkers and their families. Adult, pediatric, and prenatal care and services for chronic illness are also provided. Incumbents may be assigned to Family Practice, Women’s Health Services, or Pediatrics.

SALARY & BENEFITS

Salary: $153,415.27 – $186,477.20/year

Benefits: available to all regular Salud employees working 24+ hours per week. Part-time employees may receive some benefits on a pro-rated basis.

Additional Information:

• Employees on temporary assignments are eligible for holiday pay and California sick pay, both pro-rated based on hours worked.

DIRECT PATIENT CARE:

1. Develop a panel of patients to provide comprehensive health care in an attentive and conscientious manner
2. Perform direct patient care, including history taking, physical examination, lab and radiology ordering and interpretation, prescription of medications, immunizations and lifestyle modifications using established medical guidelines
3. Perform procedures per documented training and clinical privileges
4. Collaborate with supporting services at Salud to provide holistic care to patients, including but not limited to Community Health Services, Pediatrics, OB, Behavioral Health, Dental, etc.
5. Refer secondary conditions to specialists when appropriate
6. Institute emergency measures in life-threatening situations
7. Record all medical encounters in a timely and complete manner in the electronic health record
8. Work with scheduling and management staff to ensure provider productivity standards
9. Collaborate and/or consult with supervising physician per licensing requirements

PROFESSIONAL CONDUCT:

1. Adhere to clinic communication guidelines and expectations with internal and external customers including co-workers, patients, and external partners.
2. Communicate respectfully and effectively with people of different racial, ethnic, and cultural backgrounds and lifestyles
3. Demonstrate professional and productive collaboration with healthcare team
4. May assist with recruitment of physicians/advanced practice clinicians
5. May be asked to support learners (eg. students, residents)
6. May form liaisons with appropriate professional societies, educational institutions and community organizations to enhance clinic ability to deliver medical care to target population.
7. Adhere to departmental policies, procedures and objectives, ongoing quality improvement efforts and safety, environmental and infection control standards
8. Maintain patient confidentiality and comply with all federal and state patient confidentiality laws
9. Other duties as assigned.

Requirements

MINIMUM QUALIFICATIONS:

• Valid CA Physician Assistant or Nurse Practitioner License
• Maintain an updated CPR certificate, valid California driver's license, and valid DEA license; submit copies of renewed licenses and certificates to Human Resources.
• Board Certified or Board Qualified in Family Medicine or Adult Gerontology, preferred.

• Fluent Spanish/English.
• Bi-cultural Spanish/English
