We are looking to recruit a qualified Social Worker for the West Home First Hub based in King’s Lynn. You will hold your own caseload and be fully supported by our friendly team. Our role is to aid discharge from the acute Hospital setting and out of County hospitals for clients who are residents within the West Norfolk catchment area under the Discharge to Assess pathway.
We are an integrated multi-disciplinary team of social care professionals, health professionals, and discharge planners who triage patients and provide them with information on accessing services, aiming to support their discharge from hospital.
We are very supportive of each other; recent feedback from new colleagues in the team has been very positive regarding how they were welcomed and inducted. We encourage individual development and hold regular reflective supervision.
Your role will mainly include undertaking timely complex Care Act assessments, Mental Capacity assessment and carrying out Section 42 Safeguarding enquiries.

We offer a £2000 welcome payment for qualified and experienced Social Workers (grade J and above) new to Norfolk County Council. Amount to be repaid in full should you leave within two years of employment.
For experienced Social Workers new to Norfolk County Council, we promise to try and match or better your existing salary, providing it's in the spectrum of the grade advertised.
