Job Shift: DayEnsures the health care needs of the clients are met while attending Adult Day Services.  Provides Adult Day Services functions that reflect the Adult Day Services program philosophy and standards.
•    Assists in planning and leading individual and group activities which provide recreational and educational activities, socialization, exercise, and opportunities to express creativity and fulfill basic psychosocial needs.
•    Assists with administration of medications to clients as ordered by physician, monthly check for expired items and follow up.
•    Assists clients with personal care needs and activities of daily living (ADLs).
•    Maintains client records including clinical progress notes, client care plans, assessment schedules, vital signs, monthly summaries and physician's  examination and TB records.
•    Monitors health care needs of clients and provides follow-through as appropriate.
•    Manages emergencies in the absence of director and completes incident reports as needed.
•    Supervises and ensures safety of clients while at the center.
•    Provides supervision of Adult Day Services staff and volunteers, communicates with families, and provides program information in the absence of the Director.
•    Relates well with residents, clients, family members, staff and the community.
•    Completes training in HCSIS (EIM reporting, ISP’s), COPilotpro21 (OOA discrepancies), Critical Incident reporting
•    Assists with admission assessments and the admissions and discharges of clients according to the regulatory requirements.
•    Maintains knowledge of requirements for admissions for BHDS/ Waiver programs/ OOA
•    Documents on-line in Point Click Care of client attendance by payer
•     Prepared month-end reports and required forms for billing
•    Addresses transportation concerns, complaints and changes with schedules
•    Assists in delegation of staff assignments as changes are needed. Manages staff call offs in the absence of the director.
•    Manages client schedules and relates to payer contacts in the absence of the director.
•    Assists with inclement weather emergencies/ closures of center as needed. 

Qualifications•    Must be at least 18 years of age.
•    Current license as an LPN/RN in the Commonwealth of Pennsylvania required.
•    Prior experience in related role required.
•    Possesses good verbal, non-verbal and written communication skills.
•    Demonstrates a pleasant personality with flexibility and adaptability for frequent interruptions.
•    Proficient in use of computer and related technology required for the role.
•    Must complete Landis Homes' training for care of persons with memory loss
•    Must complete Department of Aging – Director of Adult Daily Living center training.
