POSITION SUMMARY
As an Adult Day Care Professional, you will provide direct support, case management, and program coordination for adults with disabilities in a structured day services setting.
You’ll help participants build independence, develop skills, and engage in their community while ensuring a safe, supportive, and person-centered environment.
KEY RESPONSIBILITIES
Direct Support & Programming
Provide hands-on support following individual service and behavior plans
Lead and assist with group and one-on-one activities
Support personal care, meals, and daily living skills
Encourage community outings and social engagement
Monitor health and safety and respond to concerns
Case Management & Documentation
Manage a caseload of approximately 8–10 participants
Complete documentation including case notes, progress updates, and reports
Participate in care planning and team communication
Advocate for participant goals and service alignment
Transportation & Coordination
Transport participants as needed (including wheelchair-accessible vehicles)
Assist with route coordination and daily attendance needs
Communicate schedule or coverage concerns
Team Collaboration
Follow agency policies and regulatory standards
Attend meetings and required trainings
Maintain clear communication with supervisors and team members
QUALIFICATIONS
Required:
Must be 18 years or older
High school diploma or equivalent
Experience in human services or working with individuals with disabilities
Strong communication and organizational skills
Basic computer skills (Microsoft Office)
Valid driver’s license and reliable transportation
Ability to work independently and as part of a team
Preferred:
Associate’s or Bachelor’s degree in a related field
Experience in adult day care or similar programs
Case management or service coordination experience
PHYSICAL REQUIREMENTS
Lift up to 50 lbs and assist with mobility/transfers
Push/pull wheelchairs
Ability to multitask and respond to emergencies
Work in program settings, community locations, and vehicles
Operate wheelchair-accessible vehicles and equipment as needed
