POSITION SUMMARY
As an Adult Day Care Professional, you will provide direct support, case management, and program coordination for adults with disabilities in a structured day services setting.
You’ll help participants build independence, develop skills, and engage in their community while ensuring a safe, supportive, and person-centered environment.
KEY RESPONSIBILITIES
Direct Support & Programming
- Provide hands-on support following individual service and behavior plans
- Lead and assist with group and one-on-one activities
- Support personal care, meals, and daily living skills
- Encourage community outings and social engagement
- Monitor health and safety and respond to concerns
Case Management & Documentation
- Manage a caseload of approximately 8–10 participants
- Complete documentation including case notes, progress updates, and reports
- Participate in care planning and team communication
- Advocate for participant goals and service alignment
Transportation & Coordination
- Transport participants as needed (including wheelchair-accessible vehicles)
- Assist with route coordination and daily attendance needs
- Communicate schedule or coverage concerns
Team Collaboration
- Follow agency policies and regulatory standards
- Attend meetings and required trainings
- Maintain clear communication with supervisors and team members
QUALIFICATIONS
Required:
- Must be 18 years or older
- High school diploma or equivalent
- Experience in human services or working with individuals with disabilities
- Strong communication and organizational skills
- Basic computer skills (Microsoft Office)
- Valid driver’s license and reliable transportation
- Ability to work independently and as part of a team
Preferred:
- Associate’s or Bachelor’s degree in a related field
- Experience in adult day care or similar programs
- Case management or service coordination experience
PHYSICAL REQUIREMENTS
- Lift up to 50 lbs and assist with mobility/transfers
- Push/pull wheelchairs
- Ability to multitask and respond to emergencies
- Work in program settings, community locations, and vehicles
- Operate wheelchair-accessible vehicles and equipment as needed
