
Woodfords Family Services is seeking a compassionate and qualified Adult Case Manager to join our Community Case Management team. This is a full-time (40 hours per week), salaried position offering a comprehensive and competitive benefits package.
This role is based out of our Westbrook, ME office and includes required travel throughout Cumberland County to support individuals and families in their communities.
Adult Case Managers at Woodfords play a vital role in supporting adults with intellectual and/or developmental disabilities by promoting independence, stability, and meaningful community engagement. Case Managers work collaboratively with individuals, families, and service providers to identify needs, coordinate supports, and connect consumers with appropriate resources.
Through a person-centered planning (PCP) approach, the Case Manager serves as a key advocate and communicator—ensuring services are aligned with individual goals and that all areas of development are supported with dignity and respect. This role emphasizes partnership, accountability, and empowerment to help individuals build sustainable, fulfilling lives.
