Waypoint, formerly known as Waban, is seeking skilled, motivated, and highly organized professionals to join our growing Adult Case Management team to provide services throughout Southern Maine area. Case Managers provide support to adults with intellectual disabilities/autism; serving as an advocate, ensuring choice and satisfaction with services, developing comprehensive plans of care and monitoring the individual's health and safety. Our home office for Adult Case Management in Waypoint's Southern Region is located at our Springvale, Maine location. We are seeking an Adult Case Manager to fulfill client needs throughout York County and into Cumberland County; this is a hybrid remote position after successful onboarding and new graduates are encouraged to apply. We are also seeking an Adult Case Manager to fulfill client needs throughout Greater Cumberland County, into Androscoggin and neighboring communities; this is a remote position and those with experience as an Adult Case Manager in Maine are encouraged to apply.
About Waypoint:
Waypoint offers comprehensive services, including a special purpose private school for children with autism, other developmental disabilities, and behavioral health needs, case management and outpatient therapy services for adults and children, and residential, community-based, and in-home supports for adults.
About the Role:
The Adult Case Manager assists the individual and their team of caregivers in identifying needs through a comprehensive assessment process, develops a plan of care, initiates referrals and monitors services, and ensures health, safety, and satisfaction. The Case Manager coordinates and advocates for services identified in the plan, monitors progress and evaluates the effectiveness of services through required contacts and home visits. The successful candidate is highly organized and also flexible, compassionate and also has strong professional boundaries, has strong written and verbal communication skills and utilizes a comprehensive electronic health record with ease. Requirements
Qualifications:
· BA/BS in human services or closely related field or
· BA/BS in unrelated field + minimum 1 year experience in adult IDD services
· Knowledge of the system of care in Maine preferred
Requirements:
· Reliable vehicle with proper insurance
· Valid driver’s license issued by state of residence with clean driving record
-Must be proficient in speaking, reading, and writing English with strong interpersonal and communication skills.
-Must be able to complete agency and state mandated training programs with proficiency.
Job Type: Full-time
Pay: From $25.00 per hour; higher for those with experience
