Position: Assistant Director of Rehabilitation (ADOR) / Physical Therapist (PT)

Employment Type: Full-Time

Schedule: Monday–Friday

St. Francis Healthcare Center is seeking a dedicated, compassionate, and experienced Physical Therapist (PT) to join our rehabilitation team as an Assistant Director of Rehabilitation (ADOR).

This is an excellent opportunity for a motivated rehabilitation professional who enjoys providing hands-on patient care while also supporting the coordination and day-to-day operations of the rehabilitation department.

Position Responsibilities

  • Provide comprehensive physical therapy evaluations and treatments for residents recovering from illness, injury, surgery, or functional decline.
  • Develop individualized treatment plans based on each resident’s needs, goals, and functional abilities.
  • Provide therapeutic exercises, mobility training, gait training, balance activities, and other appropriate interventions.
  • Monitor resident progress and modify treatment plans as needed.
  • Maintain accurate and timely clinical documentation in accordance with facility and regulatory requirements.
  • Collaborate with physicians, nursing staff, occupational therapists, speech therapists, CNAs, and other members of the interdisciplinary team.
  • Assist with coordinating daily rehabilitation department operations and workflow.
  • Support therapy staff with clinical guidance, scheduling, and department coordination.
  • Promote quality outcomes, resident safety, and resident-centered care.
  • Assist with maintaining compliance with applicable federal, state, and facility regulations.
  • Participate in care plan meetings and communicate resident progress with residents and their families.
  • Support a positive, professional, and collaborative rehabilitation environment.

Qualifications

  • Current California Physical Therapist (PT) license in good standing.
  • Degree in Physical Therapy from an accredited program.
  • Previous experience in a skilled nursing, post-acute, or rehabilitation setting preferred.
  • Previous leadership, supervisory, or rehabilitation management experience preferred.
  • Strong clinical, organizational, communication, and interpersonal skills.
  • Ability to work effectively as part of an interdisciplinary healthcare team.
  • Commitment to providing compassionate, high-quality, resident-centered care.
