What You’ll Do:
• Complete
Start of Care (SOC) admissions efficiently and accurately
, balancing quality with productivity.
• Perform
clinical assessments and OASIS documentation
with speed and precision.
• Initiate care plans and
quickly identify needed services and next steps
.
• Communicate effectively with the care team to
keep patient transitions moving smoothly
.
• Manage your schedule and territory to
optimize time, travel, and visit volume
.

What You Bring:
• Active
Minnesota RN license
in good standing.
• Experience with
home health admissions and OASIS
strongly preferred.
• Ability to
work independently in a fast-paced, field-based role
.
• Strong
time management and organizational skills
.
• Reliable transportation.

Additional Details:
• Mileage reimbursement provided.
• Territory-based scheduling across the Twin Cities Metro.
