Admissions Professional Nurse

St. Ann's is currently looking for an Admissions Nurse for the admissions department located on the Portland campus. The primary purpose of your job is to coordinate the resident and patient admissions from our highest volume partner to St Ann’s Transitional Care Center and assist with admissions to other services as needed.

• Has primary responsibility for maintaining relationship with our highest volume partner.
• Leads the team in organizing marketing events/meeting with primary referral to continue to build strong relations and meet expectations as requested
• Provides St. Ann’s Community tours to perspective applicants and their family members.
• Answers inquiries about SAC system from prospects who call or email.
• Conduct hospital pre-admission assessments of applicants which includes psychosocial, cognitive assessments, review of medical and financial information.
• Be proficient utilizing the electronic system hospital requires such as Care Connect, Epartner, and other various electronic systems.
• Present information regarding long term care applicants when necessary to internal stakeholders
• Review medications with pharmacy as needed.
• Coordinate room assignments, move-in process with family & Admissions Team.
• Completes pre-admission Medicare review.
• Determine Medicare eligibility/payor source for each applicant to St Ann’s Transitional Care
• Utilize electronic insurance tools (i.e. My abilities, MVP, and Excellus websites, Epaces) to determine eligibility and benefits prior to making a bed offer.
• Complete pre-certifications with insurance/determine eligibility/coverage.
• Be proficient in PDPM guidelines and coordinate efforts with the HIM and finance departments to optimize reimbursement for rehab and eligible long term care candidates.
• Provides counseling/support to applicants and family members in preparation for admission.
• Communicate decisions to applicants and/or family members; provide counseling, referrals for applicants who are not accepted.
• Discuss community options (encouraging other options within SAC) with applicants who are not appropriate for the Wegman Care Center, Leo Center for Caring, St. Ann’s Transitional Care Center, St. Ann’s Home
• Provides information and referral services to members of the community who turn to St. Ann’s Community for assistance in issues of elder care.
• Provides updated information on St. Ann’s services to community and professionals.
• Attend community events to keep abreast of current trends & promote SAC services.
• Assists with maintaining the census and running reports from SAH EHR needed.
• Implements the marketing strategies of the facility (ie. attends community health fairs, provides outreach to targeted groups, etc.)
• Responsible for achieving objectives as defined by Director of Admissions
• Completes Cherry Ridge assisted living evaluations as necessary.
• Assist prospects/family members in application, interpretation, guidance in insurance coverage and financial eligibility.
Candidate Requirements:
• Valid NYS Licensed Practical Nurse or Registered Nurse license, plus three years professional experience or equivalent combination of education and experience
• Three years’ experience in long-term care and marketing preferred.
