Sangre de Cristo Community Care is looking for a Hospice Admissions Nurse to be the first point of contact for patients and families as they begin their hospice journey. You'll help guide them with care, empathy, and expertise, ensuring they feel supported, informed, and comforted every step of the way.

Schedule: Monday-Friday. 8:30AM-5:00PM

Requirements
· Must be at least 21 years of age.
· A valid non-restricted RN license in the state of Colorado is required.
· Must have a valid non-restricted driver’s license.
· Flexibility and availability to work a weekend schedule if needed.
· At least 1 year of hospice or home health experience preferred.
