We are seeking a motivated and customer-focused Admissions Nurse (RN/LPN) to lead the admissions process and help grow facility census by providing an exceptional experience for residents, families, referral sources, and hospital partners. This role serves as a vital link between referral sources, residents, families, and the interdisciplinary team to ensure smooth and timely admissions.

Key Responsibilities

  • Coordinate and manage the admissions process from referral through admission
  • Review referrals and screen prospective residents to ensure appropriate admissions
  • Complete admission documentation, financial responsibility forms, and required paperwork
  • Verify insurance coverage and benefits eligibility
  • Maintain accurate records of bed availability and admission activity
  • Collaborate with nursing leadership, case managers, therapy, and other departments to facilitate seamless admissions
  • Coordinate transportation and required support services for incoming residents
  • Educate residents, families, and referral sources regarding facility services and admission criteria
  • Build and maintain relationships with hospitals, physicians, discharge planners, and community referral sources
  • Participate in daily stand-up meetings and census development initiatives
  • Assist with outreach events, facility tours, healthcare fairs, and community engagement activities
  • Protect resident privacy and maintain confidentiality in accordance with HIPAA regulations
  • Support census growth goals through outstanding customer service and relationship building
  • Complete additional projects and assignments as directed by leadership

Qualifications

Education & Experience

  • High school diploma or GED required
  • Bachelor's degree in Healthcare Administration, Business, Marketing, Nursing, or related field preferred
  • Minimum of two years of admissions, healthcare marketing, case management, or related experience preferred
  • Skilled nursing, rehabilitation, or post-acute care experience preferred

Licensure & Certifications

  • Current RN or LPN license
  • CPR certified
  • Experience with insurance verification and admission processes preferred
  • Knowledge of Medicare, Medicaid, and managed care plans preferred

Skills & Abilities

  • Strong customer service and relationship-building skills
  • Excellent communication and presentation abilities
  • Ability to multitask and prioritize in a fast-paced environment
  • Strong organizational and problem-solving skills
  • Experience with PointClickCare (PCC) preferred
  • Ability to work collaboratively with interdisciplinary teams and referral partners
