The Rehab Program is the Operations side of the Rehab Unit. Positions in this department include discharge planning, case management, admissions, and therapeutic recreation
Coordinates placement of all patient admissions (elective and emergent) as well as patient transfersinaccordance withclinical standards and guidelines. Promotes a sense of organizational ownership bymanagingchange and improving patient throughput. Assuresappropriate patientflow by collaboration withvarious departments, MDofficesand outlying facilities. Ensures the patient is admitted to the mostappropriate unit bedavailable using criteria for admission as well as any identified special needs.
Manages hospital bed resources and coordinates patient placement based on clinical needs, level of care, and bed availability.
Collaborates with nursing units, house supervisors, physicians, housekeeping, and patient flow teams tooptimizepatient throughput and bedutilization
Monitors hospital census, bed status, admission delays, and staffing impacts to support efficient patient flow.
Serves asa central pointof contact for patient admissions, bed assignments, and physician communication.
Identifiesand resolves bed placement issues, complaints, and process barriers while implementing continuous improvement initiatives.
Utilizes performance improvement and Six Sigma principles to enhance bed management and admission processes.
Coordinates patient admissions by reviewing clinical information, ensuringappropriate admissioncriteria are met, andfacilitatingcommunication among care teams.
Supports operational planning, scheduling, and resource allocation tomaintaineffective patient flow throughout the organization.
Requirements:
Bachelor's degree in Nursing(BSN) or equivalent combination of education and experiencerequired
Minimum of three years of direct clinical experience in an acute care hospitalrequired
Current RN licensurerequired
Telemetry experiencerequired
Critical care experience preferred.
