This is a collective bargaining position.

  • Full Time
  • Day shift (8:00 a.m. – 4:30 p.m. or 7:30 a.m. – 4:00 p.m., depending on scheduling needs)
  • 5 days/flexible

Schedule includes a 30-minute lunch. Flexibility is available, along with backup on-call support and a holiday rotation.

The Admissions Coordinator RN (Union S-RN) is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Admissions Coordinator RN (Union S-RN) is responsible for the coordination of New Life Hospice admissions process and is responsible for assisting other team members who contribute to the process.


Position Specific Responsibilities

  • • Coordinates the referral process which includes setting referrals up, gathering all necessary information for the referral nurse to make the visit including that there is an order.

    • Handles conversations with the different referral sources.

    • Initiates Plan of Care and works with the Interdisciplinary Group, and coordinates follow-up care to the patient and family as appropriate.

    • Handles all office duties including triage follow-up for case managers that are off.

    • Demonstrates education and preparation of the patient/family on dying process as evidenced by observation and/or documentation.

    • Participates in providing community and staff education.

    • Assists with the development, implementation and evaluation of Performance Improvement indicators relative to the admissions process.

    • Participates in new staff orientation.

    • Acts as primary communication link for intake and processing of all inquiries regarding New Life Hospice services.

    • Provides accurate information about all New Life Hospice programs, services and eligibility requirements.

    • Works in cooperation and collaboration with Clinical Manager to insure and facilitate the referral process.

    • Meets regularly with the Clinical Manager to ensure optimal communication and sharing of operational information.

    • Inputs data regarding demographic information on inquiries and referrals.

    • Initiates insurance verification for hospice referrals.

    • Assists in performing chart review for performance improvement monitoring.

    • Attends meetings as designated by supervisor.

    • Performs other duties as assigned.

Education and/or Experience

  • Two (2) year Associates degree in Nursing required.
  • Bachelor's degree in Nursing preferred.
  • Three (3) to five (5) years of experience in nursing.
  • One (1) to two (2) years in Home Health or Hospice nursing experience preferred.
  • Minimum of two (2) years experience in Medical-Surgical Oncology or community health nursing is required.


Skills

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.


Certifications, Licenses, and Registrations

  • Registered Nurse licensed in state of Ohio.
  • Current Ohio driver's license and automobile insurance with $100,000 - $300,000 liability is necessary.


Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
